Fees, Memberships  & Concierge Services

Bayside Medical Direct Pay Fee Schedule effective May 15, 2025

​

Service Fee Schedule for Members who are not enrolled in traditional Medicare and/or supplementing plan, as well as for those with Medicare coverage for non-covered services.

Fees may be settled either in advance or upon arrival for your visit. Payments can be made through our Patient Care Coordinator, via phone prior to or upon your in-person visit, or through the link provided in our patient portal.

Direct Pay Model

 

Bayside Medical + Med Spa fee service schedule for patient care services.

We do not process insurance claims other than Medicare and Medicare secondary supplements, however your insurance information is securely maintained in your file to facilitate communication with third-party service providers, including laboratories, imaging centers, and referral services. This allows your insurance plan to pay for services accordingly to your individual policy provisions. We strongly encourage you to seek clarification from your insurance company prior to obtaining any prescribed services. Please be advised that we cannot guarantee comprehensive knowledge of your specific policy coverage, as the details regarding covered and non-covered services may change frequently. We do accept Care Credit and any medical card for non-covered services.

Medical Visits- other than Medicare Insured 

​

  • ​30 minute New Patient visit with your provider, $150, each additional 15 minutes, $50

  • 30 minute Established Patient visit with your provider, $75-$125 (complexity based), each additional 15 minutes, $30

  • Curtesy (non-visit) lab draw collection, $30 (Including those from other providers), (when available). We will provide written orders for any necessary labs required for continuation of therapy for collection at any lab of your choice if do not choose to have your labs collected at Bayside.

  • Same Day sick Telehealth/phone/in-person visit with evaluation & prescriptions, (Established patients only) $100

  • Non-visit request e.g. (referrals for imaging, rehabilitation, specialist referrals) $50

  • ​​Non-visit medication refills for established patients (not seen in our office for 90 or more days), 1st non visit Medication refill request $50, 2 or more non-visit medication refills $90 (Subject to provider discretion). PLEASE note, it may take up to 72 hours for our provider to approve your non-visit refills, it is important to request refills in advance of your refill need.

Phone Consults for any Medical needs, Established Patients

​

  • During your initial consultations or evaluations, you will receive follow-up consultations and written recommendations, for any identified health issues or needs related to your assessment. Should you require additional follow-up consultations, either via phone or in person, please note that fees will apply at a minimum rate of $50 for every 15 minutes of consultation, at your providers discretion.

  • We will automatically post your laboratory results in your patient portal. The portal serves as a secure and reliable resource for you to access your personal health information. Your healthcare provider will follow up with you regarding any abnormal findings, providing recommendations through your preferred method of communication. You will frequently receive notifications via email or the portal for non-emergent communications; therefore, please ensure to check these resources if you have any inquiries. Additionally, you may send secure messages to your healthcare team at Bayside for any non-emergent questions.

  • We will do our best to make sure your questions are answered during your visit. Please use careful appraisal to read prescriptions, and orders to follow directions before you reach out for additional instruction. Utilize your pharmacy to inquire about medication recommendation to take with or without food, or best to take in the morning or night, etc., if not clearly indicated on your prescription.

 

Prior Authorizations

​

  • The prior authorizations (PAs) required for obtaining non-insurance covered medications or other ordered services necessitate our office to engage in multiple communications with both your insurance provider and service provider. This process often consumes 30 minutes or more of our staff’s time. Please be advised that these authorizations incur a fee of $50 per prior authorization and do not guarantee coverage by your insurance provider for the prescribed medication or ordered service (e.g., Ozempic or MRI service).

Hormone Therapy (Pellet or Non-pellet treatment)

​

  • ​Female hormone pellet therapy $350 every 4 months- Includes maintenance medication prescriptions, lab orders & review

  • Female non-pellet therapy $195 every 3 months- Includes maintenance medication prescriptions. This treatment plan includes prescriptive measures such as injections, non-injection recommendations such as peptide, topical, and oral supplemental recommendations.    

  • Male hormone pellet therapy $650 every 6 months- Includes maintenance medication prescriptions and lab orders & review

  • Male hormone non-pellet therapy is administered every three months at a cost of $350. This treatment plan includes prescriptive measures such as injections, peptide recommendations, laboratory monitoring, and lifestyle modifications to ensure optimal health outcomes.

  • On-site hormone lab collection $30 when available & by appointment only.

 

​Functional Medicine

​

  • ​Pre-Consultation $150: (optional) (Inquire about our Lunch & Lectures, Free)

  • Should you choose to engage with our recommendations, individualized cost for testing will be provided up-front before orders are written.

  • Post-consultation fee of $175, and includes testing evaluation, clinical correlation, and treatment recommendations.

  • Additional Functional medicine consultations can be scheduled in person or telehealth conference for a fee of $95 per 30 minutes.

  • We will also provide further recommended testing at a discounted rate from our laboratories to support your healing journey and help you achieve your optimal health objectives. (If necessitated).

Weight Loss & Management

​

  • ​Medical assessment $150, Includes lab orders, review of problems, dietary recommendations, individualized recommendations.

  • Monthly follow up visits for weight loss monitoring is $75. (includes written prescriptions)

  • The cost of vitamin and weight loss injections may vary based on pharmacy costs. Please consult your patient care coordinator for the latest updates. We offer a comprehensive range of resources for peptides aimed at health promotion, weight management, and support for chronic diseases.​

  • We acknowledge there may be regulatory restrictions concerning traditional (generic) compounded formulations of Ozempic, Wegovy, and Tripeptide weight loss medications. However, we offer effective peptide alternatives to these medications at a significantly reduced cost compared to their branded counterparts.

 

Vitamin Consultation

​

  • ​Full vitamin and supplement recommendations consultation (30-minute visit) $100, 45 minutes $145 (full credit given for any supplements purchased at Bayside Medical supplement dispensary).

  • ​​Vitamin Injection packages: $125-$150

  • Vitamin Infusions range $175-$550

  • Please be advised if you purchase a vitamin package and are scheduled for an injection, the allocated time slot does not permit for assessments or any additional medical requests. Therefore, a separate appointment must be arranged for any other medical services requested, unless coordinated in advance to allow time for your request. 

 

On-Demand Services

​

  • ​EKG on demand (non-emergency) $150

  • Diabetes screening $125 (Includes glucose and hemoglobin A1C test)

  • Surgical, DOT, or Clearance physical $175 

  • Fill out “paperwork” for work-school or other service starts at $75

 

Women's Membership Basic Plan

​

The Basic plan includes:

  • ​Annual 45-minute wellness visit & 30 minute follow up

  • 30-minute consultation HRT therapy (including pellets) 4 annual sessions

  • Screening recommendations & referrals (mammograms, bone density, preventative services)

  • One Functional Medicine 60-minute consultation

  • Up to 4 courtesy lab draws (for insured patients)

  • Up to 3 annual sick visits (same day or next day)

  • Extended time with provider (30-minute visits)

  • Genetic consultation with recommendations & post consultation

​Membership fees: Initial $250 fee followed by monthly enrollment fee of $175. This covers all above outlined services with for a 12-month service agreement.

Women's Membership Ultimate Plan:

​

  • ​Annual 60-minute wellness visit & follow up 45-minute consultation

  • HRT therapy (including pellets), 4 annual sessions

  • Screening recommendations & referrals (mammograms, bone density, preventative services)

  • One Functional Medicine 60-minute consultation & follow up consultation

  • Vitamin & Supplement evaluation with recommendations

  • Up to 6 courtesy lab draws (for insured patients)

  • One Annual EKG test

  • Up to 5 annual sick visits (same day or next day or Telehealth)

  • Extended time with provider (30-minute visits)

  • Weight management consultation with treatment recommendations & prescriptions.

  • Discounts on weight loss Injections if appropriate

  • Genetic consultation with recommendations & post consultation

  • Choice of One Annual Med-spa treatment, up to $1000 value.

​​Membership fees: Initial $550 fee followed by monthly enrollment fee of $275. This covers all above outlined services with a 12-month service agreement.​​​​

We would like to emphasize these fee guidelines enable your provider to deliver optimal healthcare services, including extended visits, to enhance your overall healthcare experience. We trust that this model will effectively support your health objectives.
