Content on the Site is for informational purposes only. It is not medical advice and does not create a patient-provider relationship. Do not use the Site or text messages for medical emergencies. In an emergency, call 911.

4. Appointments and Services

Booking a request through the Site does not guarantee an appointment until we confirm it. Services, pricing, memberships, and availability are subject to change. Cancellation and payment policies will be shared when you schedule.

5. Intellectual Property

All content on the Site, including text, images, and logos, belongs to Bayside Medical Practice + Med Spa or its licensors and may not be copied or reused without permission.

8. SMS Terms of Service

Program description. By opting in, you agree to receive text messages from Bayside Medical Practice + Med Spa, including appointment reminders and confirmations, post-care instructions, service notifications, and promotional offers (promotional messages only if you selected the marketing checkbox). Consent is not a condition of purchase or of receiving care.

Opt-out and help. You can cancel the SMS service at any time. Just text "STOP" to (904) 217-4550. After you send "STOP", we will send you a confirmation message that you have been unsubscribed, and you will no longer receive messages from us. Carrier liability. Carriers are not liable for delayed or undelivered messages.

Message frequency and rates. You will receive up to 4 messages per month, plus appointment-related messages as needed. Message and data rates may apply. If you have any questions about your text plan or data plan, it is best to contact your wireless provider.
